Neither number is a direct measurement of muscle tissue or strength. DEXA lean mass includes water and other non-fat, non-bone soft tissue. The most useful interpretation is usually to read LMI or ALMI beside the raw lean-mass values, regional results, fat mass, training performance, and a well-matched follow-up scan.

The practical difference

  • Total lean mass: estimated lean soft tissue across the measured body
  • Appendicular lean mass: estimated lean soft tissue in the arms and legs
  • LMI: total lean mass adjusted for height
  • ALMI: arm-and-leg lean mass adjusted for height
  • Best use: compare the same measure on the same system under similar scan conditions

How are LMI and ALMI calculated?

Both formulas divide a lean-mass result by height squared. The numerator is what changes.

Imagine a person is 1.75 meters tall. Their report shows:

  • Total lean mass: 52 kilograms
  • Appendicular lean mass: 23 kilograms

Their LMI is:

52 / (1.75 × 1.75) = 17.0 kg/m²

Their ALMI is:

23 / (1.75 × 1.75) = 7.5 kg/m²

The two values should not be compared as if one is a better score than the other. They summarize different lean-mass compartments.

Some reports calculate the indexes automatically. If yours does not, confirm whether the reported lean-mass value includes the head and which regions the software includes in appendicular lean mass before calculating it yourself.

If your report uses pounds and feet/inches

Convert height to total inches, then use the same 703 conversion factor used for other height-adjusted mass indexes:

LMI = total lean mass (lb) × 703 / height² (in²)

ALMI = arm and leg lean mass (lb) × 703 / height² (in²)

The result is still reported in kg/m². Do not read an LMI of 17 as 17% lean mass.

Why adjust lean mass for height?

Raw lean mass tends to be higher in a taller, larger person. Dividing by height squared creates an index that makes body-size context easier to discuss.

That adjustment is useful, but it does not erase every difference between people. Age, sex, population, training history, body size, scanner manufacturer, software, and the reference dataset can all influence interpretation.

For personal progress, adult height is usually stable. That means a change in LMI mainly follows the change in total lean mass, while a change in ALMI follows the change in arm-and-leg lean mass. The raw kilograms or pounds still matter because they show exactly what moved.

LMI versus ALMI

LMI and ALMI answer related but different questions.

LMI uses total lean mass

LMI includes lean tissue across the body regions counted by the report. That gives a broad view of height-adjusted lean mass, but the total also includes lean tissue in the trunk and other areas that are not skeletal muscle.

LMI can be useful when reviewing the overall balance of fat and lean mass, especially beside fat mass index. Together, the two indexes separate height-adjusted fat mass from height-adjusted lean mass in a way BMI cannot.

ALMI focuses on the limbs

ALMI uses appendicular lean mass: the sum of the lean-mass estimates for both arms and both legs. Because the limbs contain a large share of skeletal muscle and less organ tissue than the trunk, ALMI is often used in research and clinical body composition work as a muscle-related measure.

It is still not a muscle-only measurement. Hydration, glycogen, positioning, region boundaries, and other soft tissues can affect the result. LMI is not the same as lean mass percentage or FFMI

These labels can look interchangeable on a report, but the calculations differ.

  • Lean mass: the estimated amount of lean soft tissue in pounds or kilograms
  • Lean mass percentage: lean mass expressed as a share of the measured total
  • LMI: total lean mass divided by height squared
  • ALMI: arm-and-leg lean mass divided by height squared
  • Fat-free mass index, or FFMI: fat-free mass divided by height squared

Fat-free mass may include bone mineral while a DEXA lean-soft-tissue value does not. Scanner reports, fitness tools, and research papers can also label compartments differently. Before comparing two values, confirm the numerator and the units rather than relying only on the acronym.

What is a good LMI or ALMI?

There is no single gym target or universal consumer cutoff that makes an LMI or ALMI “good” for every adult.

The International Society for Clinical Densitometry lists LMI and ALMI as optional DEXA body composition measures and says their clinical utility remains uncertain. Its 2023 positions note that a low-lean-mass interpretation could use ALM divided by height squared with an appropriately matched Z-score, while consensus thresholds still need confirmation.

Reference studies also show why the comparison source matters. A U.S. study of more than 3,300 adults created age- and sex-specific LMI and ALMI percentiles from GE Lunar systems. A separate Australian study of more than 15,000 adults created lean-mass reference ranges from Hologic systems. Their authors emphasized that body composition measurements and suitable reference values can differ by scanner system and population.

A practical reading sequence is:

  • Confirm the label and unit. Check whether the report shows LMI, ALMI, FFMI, lean mass, or a percentage.
  • Use the report's matching reference. Prefer a reference that fits the scanner, software, age, sex, and population context when one is provided.
  • Read the raw values. Review total lean mass and arm-and-leg lean mass beside the indexes.
  • Compare your own trend. Use a consistent baseline rather than chasing someone else's number.
  • Add performance context. Strength, training volume, measurements, and function answer questions the scan cannot.

How to compare LMI and ALMI between scans

The indexes become more useful when the scan conditions and measurement system stay consistent.

  • Use the same scanner and software when possible. Lean-mass estimates can differ across systems.
  • Match preparation. Keep time of day, hydration, food, recent training, clothing, and creatine routine reasonably similar.
  • Check positioning. Arm, leg, and region placement matter especially for ALMI.
  • Read the raw mass change. A change in the index should agree with the corresponding total or appendicular lean-mass value.
  • Review regional results. ALMI can rise even if the change is concentrated in one limb or region.
  • Do not overread a tiny shift. Measurement precision and normal day-to-day biology can be larger than a very small difference.
  • Use enough time. A full training, fat-loss, or maintenance phase is usually more informative than closely spaced scans.

What LMI and ALMI cannot tell you

LMI and ALMI cannot show:

  • Whether a change is pure new muscle tissue
  • How strong, powerful, coordinated, or mobile you are
  • Which individual muscle changed
  • Why a value is higher or lower
  • Whether a small one-scan difference is meaningful
  • Whether you have sarcopenia or another medical condition
  • What training, nutrition, or treatment plan you need

Those limits do not make the indexes unhelpful. They define the right job for them: summarize lean-mass measurements, add body-size context, and support a broader progress conversation.
